Privacy Policy

Creative Health Solutions
Effective date: October 21, 2026

This Privacy Policy explains how Creative Health Solutions ("we," "us," or "our") collects, uses, shares, and protects personal information when you visit www.chsquotes.com (the "Site"), request a quote, chat with us, schedule a call, or communicate with us by phone, email, or text message.

1. Who We Are

Creative Health Solutions is an independent health insurance agency. We help individuals, families, self-employed people, and small businesses compare and enroll in health insurance and related coverage.

Creative Health Solutions

Melbourne, FL 32904

2. Information We Collect

Information you give us through our forms

When you request a quote on the Site, we collect the information you enter, such as:

  • First and last name
  • Email address
  • Phone number
  • The type of coverage you are interested in (for example, individual or family, self-employed, small business or group, Medicare, or life insurance)
  • Anything you choose to tell us about your situation in the message field
  • Your text message consent choices, if you check either SMS consent box

Health and coverage details. To prepare a quote, you may choose to share details such as your current coverage, budget, the people who need coverage, their ages, your ZIP code, your doctors, your prescriptions, or health information. We collect this only when you choose to share it, and we use it to quote, compare, and help you enroll in coverage. Please do not include Social Security numbers, medical records, or payment card numbers in website forms or chat.

Information collected through website chat

If you use the chat feature on the Site, we collect the information you enter in the chat, such as your name, phone number, email address, your messages, and any consent choices you make in the chat window.

Information collected when you schedule a call

We use Calendly to schedule appointments. When you book a call, we receive the information you enter, which may include your name, email address, phone number (if you choose a phone call or text reminders), ZIP code, whether you are self-employed or a business owner, who needs coverage, ages and gender, and any notes you add. Calendly's own privacy notice also applies to information you give through Calendly.

Information you give us when we quote or enroll you

If you ask us to prepare quotes or help you apply for coverage, we may ask for more information that the insurance company or the Health Insurance Marketplace requires, such as dates of birth, household members, estimated income (for subsidy eligibility), and other application details. We ask only for what is needed for the quote or application you requested.

Communications

We keep records of your calls, emails, and text messages with us, so we can respond to you and keep your file up to date.

Cookies and analytics

When you visit the Site, we and our service providers automatically collect certain information through cookies and similar technologies. This includes your IP address, browser and device type, pages viewed, links clicked, and the date and time of your visit. We use:

  • Cookies set by our website platform (Squarespace) to operate the Site
  • Google Analytics, to understand how visitors use the Site
  • Google reCAPTCHA on our forms, to protect against spam and abuse
  • Embedded features, such as our scheduling tool and Google reviews display, which may set their own cookies

UTM and attribution data

When you arrive at the Site from an ad, email, or other link, we record campaign and attribution details. These include campaign tags (such as utm_source, utm_medium, utm_campaign, utm_term, and utm_content), ad click identifiers (such as gclid, wbraid, and gbraid), the referring website, the first page you landed on, and when you first and last visited. We store this in a first-party cookie and in your browser's local storage for up to 90 days. It is attached to any quote request you submit, so we can understand which marketing channels people use to find us.

3. How We Use Your Information

We use the information we collect to:

  • Respond to your quote request, chat, or appointment
  • Prepare and compare insurance quotes and help you apply for or enroll in coverage you choose
  • Contact you by phone, email, or text message (texts only if you opted in) about your inquiry, appointments, and coverage
  • Send marketing communications, including marketing text messages only if you gave separate consent to receive them
  • Provide ongoing service, such as renewal reminders and help with your policy
  • Understand how the Site is used and measure our marketing
  • Protect the Site and our business against fraud, spam, and misuse
  • Comply with legal, regulatory, and insurance licensing requirements

4. How We Share Your Information

We do not sell your personal information. We share it only as described below.

Service providers. We use service providers who process information for us so we can run our business. They may use it only to provide services to us. They include:

  • Website host: Squarespace, which hosts the Site and stores form submissions
  • CRM and messaging platform: our customer relationship management system, which we use to manage contacts and to send and receive email and text messages, including the website chat feature
  • Scheduling: Calendly, which we use to book appointments
  • Lead-routing webhook provider: a service that receives quote-form submissions from the Site and delivers them to our CRM
  • Analytics and security: Google Analytics and Google reCAPTCHA

Insurance companies and the Health Insurance Marketplace. We share your information with insurance companies, or with the Health Insurance Marketplace, only when you ask us to quote or enroll you in coverage, and only the information needed for that request.

Legal and safety reasons. We may disclose information if required by law, regulation, subpoena, or court order, or to respond to a request from an insurance regulator. We may also disclose it to protect the rights, safety, or property of our clients, us, or others.

Business transfers. If our business is sold, merged, or transferred, information may be transferred as part of that transaction, subject to this Privacy Policy.

None of the sharing described in this section includes text messaging originator opt-in data or consent. See Section 5.

5. Text Messaging (SMS) and Mobile Information

If you provide your mobile number and opt in to receive text messages from Creative Health Solutions, we use your number to send the messages you requested, such as insurance quotes, appointment reminders, and follow-ups about your inquiry.

No mobile information will be shared with third parties or affiliates for marketing or promotional purposes. Mobile phone numbers and SMS opt-in data and consent are never sold, rented, or shared with any third party or affiliate for their marketing purposes. All of the information-sharing categories described in this Privacy Policy exclude text messaging originator opt-in data and consent; this information will not be shared with any third parties.

We may share your information with service providers who help us run our business and deliver messages (for example, our CRM and SMS messaging platform). They may use it only to provide those services to us, not for their own marketing.

You can opt out of text messages at any time by replying STOP. Reply HELP for help, or contact us at (321) 872-4940 or dmills@chsquotes.com. Message frequency varies. Msg & data rates may apply. For details, see our SMS Terms.

6. Data Retention

We keep personal information for as long as we need it for the purposes described in this Privacy Policy. That includes serving you as a client, keeping records required by insurance and other laws, resolving disputes, and enforcing our agreements. When we no longer need information, we delete it or make it anonymous. Records of your text message consent and opt-out requests are kept for as long as needed to show compliance.

7. Security

We use reasonable administrative, technical, and physical safeguards to protect personal information, and we limit access to people and providers who need it. No method of transmission over the internet or of electronic storage is completely secure, so we cannot guarantee absolute security.

8. Your Choices and Rights

  • Text messages: Reply STOP to any text to opt out. Reply HELP for help. You can also contact us to change your text message preferences.
  • Email: Use the unsubscribe link in any marketing email, or contact us.
  • Phone calls: Ask us at any time to stop calling you.
  • Cookies: You can block or delete cookies in your browser settings. Some parts of the Site may not work properly without them. You can opt out of Google Analytics with Google's browser add-on.
  • Access, correction, and deletion: You may ask us for a copy of the personal information we hold about you, or ask us to correct or delete it, by contacting us using the details in Section 13. We may need to verify your identity before acting on your request. We may keep some information when the law requires us to, or for legitimate business purposes such as records of insurance transactions.

Opting out of marketing does not stop messages we need to send about a request you made or coverage we are servicing for you, unless you also ask us to stop those.

9. Children's Privacy

The Site is not directed to children under 13, and we do not knowingly collect personal information directly from children. If a parent or guardian gives us information about a child to quote or enroll family coverage, we use it only for that purpose. If you believe a child has given us personal information, contact us and we will delete it.

10. Links to Other Sites

The Site may link to other websites and services, such as HealthCare.gov, Medicare.gov, insurance company websites, Calendly, and Google reviews. We are not responsible for the privacy practices of those sites. Please review their privacy policies.

11. Changes to This Privacy Policy

We may update this Privacy Policy from time to time. When we do, we will post the updated version on this page and change the effective date above.

12. Related Terms

Our Terms and Conditions, including our SMS Terms, also apply to your use of the Site and our text messaging program.
